Error in Quiz 3a and 3b

The question that asks you to approximate a square root may not have any choices with the correct answer. Not all versions of the quiz exhibit this problem. If your quiz was defective you have two options:
  1. Count the defective quiz only out of nine questions, or
  2. Take an extra one question quiz to replace the defective question.
The question that was defective on some exams has the form:
Approximate $\sqrt{36.977}$ using the formula

    $f(a+h)\approx f(a)+f'(a) h$

where $f(x)=\sqrt x$ and $a=36$.

A) 6.080871648
B) 6.081416667
C) 5.918583333
D) 6.079371648
E) 6.082916667

(100) ANSWER 6:
(A) (B) (C) (D) (E)
The correct answer to the above problem is
    $$\sqrt{36}+{1\over 2\sqrt{36}}(0.977)=6.081416667$$
which, in this case, is listed among the choices. If your quiz contained a defective version of this question, please let me know and tell me which of the options to fix it you would prefer.
